Incidence trends and survival of salivary gland cancer in the provinces of Girona and Tarragona (1994-2018): population-based study

Background: salivary gland cancer is a rare type of cancer and usually studied within the context of head and neck cancer. This study aims to provide a regional approach of SGC incidence trends and survival overall and by histologies, which may afford knowledge for future studies. Objectives: To determine the incidence, trends in incidence and survival of SGC, both overall and by histological subtypes based on the International Classification of Diseases for Oncology, 3rd edition (ICD-O-3) morphologic codes, while also discerning variations by sexes between 1994 and 2018 in the population of the provinces of Girona and Tarragona. Design: A population-based retrospective observational cohort study conducted between May 2024 and February 2025. Method: 334 cases of patients diagnosed with primary SGC from 1994 to 2018 were identified from the Cancer Registry of Girona (CRG) and Cancer Registry of Tarragona (CRT) database to estimate the incidence and trends. Incidence has been estimated as Crude Rate (CR), Age-Standardized to the European population Incidence Rate (ASIRE), Age-Standardized to the World population Incidence Rate (ASIRW) and Age-specific rates (ASR). Incidence trends for overall SGC and by histology were calculated subsequently through the Annual Percent Change (APC) of ASIR, expressed as a percentage increase or decrease with a 95% confidence interval. 148 cases of patients diagnosed with primary SGC from 1994 to 2018 were identified from CRG database to estimate observed and net survival by Kaplan-Maier and Hakulinen method respectively. Results: A significant decrease in incidence, with an APC of -2.22, was observed between 1994 and 2018 in overall SGC. Incidence also exhibit decline for poor defined neoplasms, with an APC of -5.22. Among men a significant decrease with an APC of -2.78 was observed. Overall survival after 5 years diagnosis was around 53%. Men overall OS was 48% and women 58.3%. Infiltrating duct carcinoma had the worse survival with 16.7% at 5 years diagnosis. Conclusions: These data suggest a similar distribution of histologies compared to other cohorts with a significant decrease in the incidence of overall SGC, along with the poor defined neoplasms in Girona and Tarragona from 1994 to 2018. The survival is determined by the histology subtype
